The thyroid gland is a butterfly-shaped shaped small gland found in the windpipe in the lower neck region. It produces Thyroxine (T4) and Triiodothyronine (T4), two major hormones in the body. These two combinedly help in controlling the rate at which the body consumes energy. The Free Thyroxine (FT4) test measures the total amount of T4 in the blood.
The T4 hormone is found relatively inactive, but it is transformed to the much more active T3 when reaches the liver or other tissues. The feedback system that our body uses to maintain the right quantity of Thyroid hormones in the blood regulate the T4 levels. The feedback system also includes T3 and TSH which stands for Thyroid Stimulating Hormone, along with TRH (Thyroprotein Releasing Hormone), which is a product of hypothalamus.
In response to the TRH stimulation, the pituitary gland produces Thyroid Stimulating Hormone or TSH after the decrease in the levels of thyroid hormone in the blood. The counteraction of TSH is to stimulate the thyroid by binding the TSH receptor to release T4 and T3.
When the blood gets enriched of hormone levels. The pituitary gland regulates the TSH production and produces less amount of TSH, and the Thyroid gland also produces less T4 and T3 in response.
Symptoms of hypothyroidism like weight gain, cold intolerance, irregular menstruation, dry skin, and fatigue are seen in the person having the insufficient T4 and T3 production (underactive thyroid) in the body. These problems occur due to thyroid dysfunction or insufficient TSH.
Overactive Thyroid leads to a disease called hyperthyroidism. Symptoms such as anxiety, weight loss, sleeping difficulty, rapid heart rate, irritated eyes and in some cases, bulging eyes, puffiness around dry and tremors are related to hyperthyroidism. The Free Thyroxine (FT4) test detects the effect of these conditions on the thyroid hormone production.
The test result can show levels of free T4 hormones to be high, low or in the range. The high free T4 results mean an overactive Thyroid Gland which is known as Hyperthyroidism. The low free T4 levels point to Hypothyroidism or an underactive thyroid gland.
Free Thyroxine Index or FTI is used to estimate the free T4 concentration. FTI which is sometimes referred to as T7 is a calculated value determined from the total T4 test along with some evaluation of the level of thyroid hormone-binding proteins.
No need to prepare for the test. The FT4 test can be undergone anytime. One thing to keep in mind is that if you are under medications, then please let the test provider know about that. Sometimes there are some medicines which may affect the test results.
